VTL Equipment Check Out
Students must fill out the form below to check out equipment from the VTL. ALL fields must be filled out. Any item left blank may result in the denial of checkout application. Equipment is reserved on a first come, first serve basis. Students who have a completed form will have priority over those who's forms are not fully completed. The VTL equipment checkout policy, as of Winter 2011, requires that all equipment must remain on campus. Students may reserve equipment for no more than 10 hours. Checkout time starts at 10am and ends at 8pm Mon - Fri. Equipment is also available on Saturdays from 10am to 3pm. If equipment is needed for more than the allowed time permits, Students can re-checkout gear the following day, providing it is not previously reserved. Once you have completed this form you MUST also print a copy that can be used with your instructor when checking out the equipment |

Remember to keep a copy of the checkout form with you. Use the checkout form to catalog all the pieces of equipment so that it is packed properly and no items are lost before turning your packages in. If any items are lost or broken when checked in, it is the Student's responsibilty to replace those items.
The VTL is NOT responsible for providing batteries. If you need AA or 9v batteries you must supply your own.
